Everything you need to gather for your Florida residency process
Original Documents Required
You must have the actual, original physical documents listed below. Photos, photocopies, screenshots, and digital images are not accepted. Please gather all required documents before proceeding.
USPS Form 1583
Before we can assign you a Florida address, the USPS requires a completed Form 1583 (Authorization to Receive Mail). You will need to present your original documents in front of a notary public to complete this step.
You must bring the physical, original documents below to your notary appointment. Digital copies will not be accepted.
One Photo ID (required)
- Passport
- U.S. State, Territory, or Tribal Driver's License or ID Card
- Uniformed Service ID
- U.S. Access Card
- U.S. University ID Card
- Matricula Consular
- NEXUS Card
- Certificate of Naturalization
- U.S. Permanent Resident Card
One Proof of Address (required)

Florida Driver's License
Once we assign you a Florida address and provide your welcome letter and Florida home agreement, you can apply for a Florida driver's license at the Sumter County Tax Collector's office. Surrendering your previous state's license is one of the strongest signals of Florida domicile intent.
All residential address documents must display your new Florida address.
Primary Identity (one required)
- Original or certified copy of your U.S. Birth Certificate (county health department or Bureau of Vital Statistics only; hospital certificates are not accepted)
- Valid U.S. Passport or Passport Card
- Consular Report of Birth Abroad
- Certificate of Naturalization (Form N-550 or N-570)
- Certificate of Citizenship (Form N-560 or N-561)
Legal Name Change (if applicable)
If you are using a birth certificate and your legal name has changed, you must provide proof of the name change.
- Marriage Certificate (must be an original state-issued or certified copy)
- Court-ordered name change document
Proof of Social Security Number (one required)
The name on your Social Security record must match the name that will appear on your Florida driver's license.
- Social Security card (must show your current legal name)
- W-2 form (not handwritten)
- Pay stub showing your full name and Social Security number
- SSA-1099
- Any 1099 form (not handwritten)
Two Forms of Proof of Residential Address
You need two different documents showing your Florida residential address. Documents must be dated within 60 days unless otherwise noted.
- Your Tax Base welcome letter (provided by us upon enrollment)*
- Your Tax Base Florida home agreement (provided by us upon enrollment)*
- Deed, mortgage, monthly mortgage statement, or residential lease agreement
- Florida voter registration card
- Florida vehicle registration or title
- Utility bill (water, gas, electricity, cell phone, or cable)
- Homeowner or automobile insurance policy or bill
- Financial statement (bank, credit card, or investment account)
- Government agency mail or documents
- Employer documentation (W-2, 1099, or paycheck stub)
- Medical or health card/bill
*These documents are routinely accepted as proof of residential address in Sumter County. While we have never encountered an issue, acceptance is ultimately at the discretion of the issuing office.
Declaration of Domicile
After obtaining your Florida driver's license, we recommend filing a Declaration of Domicile with the Sumter County Clerk of the Circuit Court. This is a sworn statement declaring Florida as your permanent home and is one of the key legal steps in establishing domicile.
What you'll need
- Your Florida residential address (provided by Your Tax Base)
- A valid photo ID (your new Florida driver's license works perfectly)
- The Declaration of Domicile form (we pre-fill this for you during enrollment)
- Notarization (the form must be signed before a notary public, available in person or online)
- A small filing fee (typically under $15, varies by county)
Voter Registration
After filing your Declaration of Domicile, registering to vote in Florida further strengthens your domicile claim. You can register at the Sumter County Tax Collector's office (often at the same time you get your driver's license), online, or by mail.
What you'll need
- Your Florida driver's license number OR the last four digits of your Social Security number
- Your Florida residential address
Recommended Order
The Form 1583 is required to receive your Florida address from us. From there, we recommend getting your driver's license, then filing your Declaration of Domicile, and finally registering to vote. Each step builds on the previous one and strengthens your Florida domicile.
